You are missing something.  The "ears" are what your hands grab to remove
the cap, they are not internal.

Now I am a little confused... (situation normal)... how would I get a 4 ear 
cap on if the neck of the tank only has 2 cut aways for the ears to go down 
into?
Or am I mising the point of something?
