I recall an early Ferrari experiment with a vertical twin (2500 cc?) to use at places like Monte Carlo were torque would be enhanced in theory but the test engine was determined to vibrate excessively. I dont recall it made it into a chassis? I assume counter-rotating balance shafts hadn't come along yet?
Some designers do single cylinder mockups to test engine layouts but usually those aren't operational. I have seen photos of V, W and H mockups with a cylinder on each bank.
