Please allow me to add possibility number four;
(4) Corrosion or wiring within the connector is causing intermittent or NO
operation.
As good as the Bosch connectors are, they do fail.
(I experienced this on an E30 M3.)

There are three possibilities here.
1) The Thermotime switch is bad.
2) The Thermotime switch is miswired. (I'm not sure
if the resistance of the heater is low enough to
allow actuation of the CS injector)
3) The power application to the Thermotime switch
heater element during "start" isn't happening.
I'm betting on #1 myself
