I doubt that the fine would hurt a billionaire that much ... seems like a lot to you and I, but a billion is 1,000 million. 100 out of 1,000 isn't that much. It's all perspective!

Suppose McL refuses? I doubt that there is much legal punch behind the decision, as it would sort of be like your country club fining you $100 for peeing into a tree in a public place (don't ask......). The only enforcement is total exclusion until the amount is paid (again, don't ask......). So....
Far fetched thought: Maybe McL refuses to pay. Which means leaving F1. What would they do? As I recall, their roots were in sports cars. Remember the McLaren CanAm cars? M8F, if my soggy brain cells remember, was a designation for a real ground pounder using an all aluminum 530cid Chevy. Fast forward and look to the ALMS and the sister Euro series. Yes, I know that it is a drop in the bucket compared to F1, but it still is interesting racing, involving Audi, Pugeot (sp), and even Porsche.
