This was a US only thing. Euro cars were more simple.There is a sensor on the coolant tank of the (US) injected QV's and 328's. It's been awhile but as I recall, the switch opens when the temperature rises. When cold, the switch is closed and energizes a vacuum switch which is partially responsible for raising the idle speed.
There is also an oil temperature switch which works in a similar fashion and when it closes tells the system to go into closed loop mode.
The other part of the fast idle puzzle is the AAV (aux. air valve), it's possible that this failed or more likely, you have a disconnected or cracked air hose.
