The new STANDARD for emergency personnel is "ICE" - that is what emergency personnel are now trained to look for. You can keep the HOME entry but should add ICE.
You don't want to add a challenge the emergency worker to figure out your "system" - particularly under the stress of the type of emergency situation where you are likely to be unresponsive - and should, instead, adopt THEIR STANDARD.

You should, right now, add a contact to your cell phone under the name ICE - "In Case of Emergency." Put in the contact information of the person or persons that you would want contacted should you be in an accident that renders you unable to speak or otherwise provide your own medical direction.
Make sure that person has pertinent information about your medical history, medications, and medical directives (and care of your animals).
Now-a-days, everyone carries a cell phone and for the past few years EMTs have been trained to look to the phone and check for ICE information when confronted with a non-responsive patient.
This can save your life.
