Stories like this make you wonder what health care would be like if there
was no such thing as health insurance at all. Maybe, just maybe, health
care actually would be affordable for 95% of people and medicaid/medicare
could take care of the truly indigent.

I do think that it is true that people today do not have any real incentive
to investigate costs of their health care because they only pay a low
deductible or copay no matter what the total charge for service is. The
insurance companies might have an incentive to try to lower costs but they do
not
have the "feet on the ground" ability to chase down individual claims even
though they do set prices on what they will pay for service. It is just
easier to pass along the costs of the low percentage of insured people who
really incur the high costs for surgeries or treatments to the vast majority
who are lucky enough not to need those services.

Insurance was created in order to spread the high costs of low probability
events across a large population who would be willing to pay for protection
against the low probability event that would have catastrophic costs to
them if it happened. It has evolved to where everyone expects that costs
that are incurred by 100% of the population should also be covered and that
just makes no sense since there is no spreading of those risks (costs) when
it happens to 100% of the population. This just adds layers and layers of
bureaucracy and additional costs and opens up the possibility of greater
fraud and waste.

I think that health insurance costs and the costs of medical care would go
way down if the government got out of the way and insurance deductibles
were raised so that routine everyday care was paid for by the individuals.
Leave health insurance for catastrophic coverage, not for a wealth transfer
or welfare system payment.

I had a friend in Las Vegas whose wife had cancer treatments. There

reduced the bill 50% because they wanted to pay cash.
